Stage IV is divided into stages IVA, IVB and IVC. In stage IVB, is one of the following situations: 1) the cancer has spread to the space in front of the spine and surrounding the carotid artery, or has spread to parts of the chest and may have spread to one or more lymph nodes anywhere in the neck (lymph nodes may be any size), 2) the cancer has spread to a lymph node that measures more than six inches and may have spread as far as into the space in front of the spine, around the carotid artery or the chest.
